Health Savings Account (HSA) benefits cards provide a simple and convenient way to pay for eligible medical, dental, and vision expenses directly from your account. The card works like a debit card, allowing you to access your funds at the point of service—eliminating the need to pay out of pocket and submit reimbursement requests later.
This guide explains how your benefits card works on the Elevate platform and answers common questions about transactions and PIN usage.
What account does a debit card transaction withdraw money from?
Your benefits card may access more than one account, including:
- A limited purpose health FSA (for dental and vision expenses), and
- An HSA (for eligible medical expenses).
When the card is used, the system determines which account to use based on the merchant category code (MCC) assigned to the provider.
Transactions follow this priority:
- The card first attempts to pay from the limited purpose health FSA.
- If the provider’s MCC is classified as dental or vision, the expense is paid from the limited purpose health FSA.
- If the MCC does not match a dental or vision provider, the transaction is paid from the HSA.
Why did a dental or vision charge come out of my HSA?
The card relies entirely on the merchant’s MCC. If a dental or vision provider is using an incorrect or non-standard MCC, the transaction will default to the HSA instead of the limited purpose health FSA.
What should I do if a charge pulls from my HSA instead of my limited purpose health FSA?
If HSA funds are used for qualified medical expenses, the distribution is not taxable. See articles here and here. You may choose to:

Apply the HSA distribution to other eligible medical expenses and report the use on your tax return,
or
- Submit a claim with your dental or vision receipts to be reimbursed from your limited purpose health FSA. Reimbursement will be issued by check or direct deposit.
If you would like to return the funds to your HSA:
- Complete a distribution reversal form.
- Submit the form along with a check for the amount you wish to return.
- Submit a claim for reimbursement from your limited purpose health FSA using the applicable receipts.
What should I do if a charge pulls from my limited purpose health FSA instead of my HSA?
Because an HSA is an individually owned account, funds cannot be transferred directly from an HSA to a limited purpose health FSA. If this occurs:
- Take a distribution from your HSA (by check or electronic transfer).
- Send a check to Rocky Mountain Reserve for deposit into your limited purpose health FSA.
Debit Card PIN FAQ
You may receive a benefits card for your HSA, FSA, limited purpose FSA, dependent care account, or commuter benefits. In most cases, the card should be run as credit, though some merchants require a 4-digit PIN.
The merchant is asking for a PIN number for my RMR debit card.
The Dodd-Frank law requires all benefit cards to be assigned a PIN. Some merchants require the PIN to process the transaction, while others allow the card to be run as credit. Always try running the card as credit first.
Do all merchants require a PIN number?
No. Many merchants allow benefits cards to be processed as either credit or debit. Merchants may prefer debit transactions because credit transactions carry higher processing fees.
How do I know if a PIN is required?
If the transaction processes as credit, no PIN is needed. If the merchant requires debit processing, you will be prompted to enter your PIN.
How do I find my PIN?
To set your PIN:
- Call 877-315-3483 and select option 2

Follow the prompts to create your PIN
You will be asked to enter:
Your 16-digit card number
Use the number on the front of the card you are activating or setting a PIN for.Your ZIP code
This must match the primary address on file for your account.The last 3 digits on the back of your card (CVV)
Use the CVV from the card you are activating or updating.-
Your date of birth (MMYY format)
Enter the primary cardholder’s date of birth in MMYY format only.
If you are setting up a PIN for a dependent card, you must still use the primary cardholder’s date of birth.⚠️ Important reminder
Your benefits card cannot be used to withdraw cash at ATMs or point-of-sale terminals.
This is an automated phone system used only for PIN setup. It is not Rocky Mountain Reserve’s customer service line.
Can I set my PIN online?
No. For security reasons, PINs can only be set by phone.
Can Rocky Mountain Reserve access or set my PIN?
No. Rocky Mountain Reserve does not have access to card PINs.
